The base game is stellar for the first 25 screens, then the difficulty escalates for the final 5, then jumps significantly higher during extra.
Despite the recycled setup jumps in the base game, the platforming manages to be quite creative. Nothing gets harder than about 60 difficulty, and for the most part stays between 50 and 55. I wish gimmicks would have been used more frequently, however, both to help break up the repetitiveness of some of the platforming and to introduce a different challenge. The apple platforms are severely under-utilized, which I find incredibly disappointing, especially since the platform segments which feature them are quite good.
I had a similar experience to Lucien on floor 49, though I'm not sure if I played the pre- or post-nerf version. I didn't notice any weirdly precise jump on floor 46, so perhaps I played the post-nerf version, or maybe I got lucky.
The extra screens have volatile difficulty. The only screens I outright disliked were 36–40, which felt very cramped and awkward. I also wasn't a fan of 49, for the reason mentioned above.
The regular clear screen has a song that I vibed with immediately, so much that I sat there and listened to the whole thing, then looked it up on YouTube. Just figured I'd add that in.
If you prefer to have fun while playing needle, then stick with the base game. Or if you're really itching for some harder needle and can handle high-60s without getting stuck too often (or at all), maybe consider checking out extra as well, but bear in mind that it's lower quality than the base game.
