This is a 30 floor game, with just one save per screen. As such, the difficulties - Medium, Hard, and Very Hard - do not alter the save placement, but rather the needle design itself. My ratings are based on playing Very Hard, though Hard is the creator's recommended difficulty.
Do not play this game on Very Hard. It acts as an excellent argument against fangames including multiple distinct difficulty modes, because all Very Hard does is buff the existing platforming in obnoxious ways without any consideration for how these buffs impact the overall flow of a save. More specifically, it features drop gates, T-bones, sideways gates, and 16px gaps strewn haphazardly across minute-long saves, resulting in many floors becoming what I can only describe as a "choke nightmare." At the same time, the buffing isn't even really that consistent, so there were a few floors I beat on my first try despite spending 10-15 minutes on the previous one...
Hard mode seems to be a pretty smooth experience, so just play that if you're interested in this game. I assume the other reviews on here are based on Hard mode.
The boss at the end is based on SaGa Frontier and is trivial if you know Japanese, as it announces each attack in advance at the top of the screen. If you don't know Japanese, I guess you should learn it.

Incredible level design, production values, and basically everything. There's a ton of content and it's all as inventive as it is fun to play. The creativity on display is stunning, and even when I feel the gameplay stumbles on the fun factor (Acid Bog), the gimmicks are still interesting enough to keep me entertained. Overall, easily one of the best fangames ever made. Personally I wasn't too fond of some of the bosses (Ice Guardian) and tend to dislike avoidances in general, but I won't dock points for that because the bosses themselves (except Ice Guardian?) are fine, they just aren't the kind of thing I personally enjoy playing. I recommend this game to basically everyone, as long as you can handle the pretty hard (but not too hard) needle.

Really fun needle game with very few generic jumps and consistently satisfying saves. No real complaints outside of some silly save balance issues here and there. Play it.
